Metro Care Human Services provides a variety of high quality mental health counseling including intervention, case management, advocacy, rehabilitative services to adults, relocation services, and Moving Home Minnesota (MHM) assistance.
We meet the needs of individuals experiencing serious mental illness and their families by addressing the individual’s physical, mental, spiritual, substance abuse and wellness goals.

We work with eligible nursing facility residents to locate housing in the community and put supports in place as they transition into their new home. Metro Care Human Services is a new culturally sensitive organization with the purpose of providing psychosocial and mental health services to diverse adult minority populations.
